Requirement:
The window shows the parameter index and name to be set. It shows fields labelled with the type of attributes or qualifiers to be supplied by the user. It provides empty fields for the user to specify the values to be set.

The OK is used by the use to indicate the information has been supplied. The CANCEL is used to indicate no change of the parameter qualifiers and or attributes is required.
